Medication Order
  • All the medication orders for patients in ward will be prescribed by doctors using the self-carbonised Medication Chart consists of two separate sheet; a white sheet (Original prescriptions where doctor write their orders) and the yellow carbonised sheet.
  • The original written prescriptions (white sheet or termed as “cips“) will be separated the carbon copy (yellow sheet) before they are being sent to the Satellite Pharmacy. Usually, the original order in the white sheet will be sent to pharmacy department while the yellow sheet will be retained at the nursing unit for recording of drug administration to patients.


  • Basically, a new medication order (prescription) will be made in the following situations:
    • Patients who are newly admitted
    • Patients who have undergone an operation or special procedures
    • Patients who are transferred from other wards
    • A condition where the treatment needs to be continued but the prescriptions are expired

  • Any medications for patients who are currently in the ward must be made through a prescription, hence, any changes in dose, dosage form, frequency or treatment duration warrants a new prescription attached with an Advice Note.
  • The prescription of patients who have just been admitted into the ward has to be sent with Cumulative Medication Record (CMR).
  • It is the responsibility of the nurse on duty to send prescription for all the patients in the ward to the pharmacy unit. Under certain circumstances, Penolong Perawatan Kesihatan (PKK) can be directed by the nurse to send the prescription to the pharmacy.
