Charting Cumulative Medication Record
All new prescriptions should be pasted on the patient’s CMR after screening and the expiry date of the prescription recorded in the amount column. Once each prescription is screened and enquiries are confirmed with prescribers, the pharmacists will start doing the charting on the CMR.

For example:


The prescription at the CMR must be stamped with “UBAT DIBERHENTIKAN“ to notify discontinuation of a medication. At the Medication Chart (yellow carbon copy) in the ward, the prescriber/pharmacist/nurse should also record the discontinuation of the medication.

To withhold a medication, it must be recorded as “WH” in the CMR at the amount column. It should be also recorded in the Medical Chart (yellow carbon copy) in the ward by the doctor/pharmacist/nurse.

Stamping with “RUJUK PRESKRIPSI BARU” is needed when there are changes in medication dose or administration frequency.
