Information To Be Delivered To Patients During Counselling Session

It is important for the pharmacists to accurately identify the important information required by patients to maximize their chances of solving or preventing their drug related problems. The information that must be provided to patients:
  • Generic and brand name of the drug
  • Indication of the drug and expected outcome of the drug
  • Strength of medication prescribed
  • Dose of the drug that must be taken
  • Route of administration of the drug (Example : orally or sublingually)
  • The onset of action of the drug
  • Duration of therapy
  • Warning and precautions of the drug
  • Important side effects that the patient might possibly get
  • Ways to minimize the side effects including the needs to contact the prescriber or pharmacists
  • Non pharmacological treatment
  • Action to be taken in the event of a missed dose.
  • Drug-drug, drug-food, drug-alcohol and drug-test/procedure interactions.
  • Storage recommendation and important instructions
  • Information on how to use devices (Example : Inhaler for asthmatic patients)
